Virtual Nurse Practitioner - Remote
Seeking a clinically confident Nurse Practitioner to join its virtual clinical team supporting a high-risk, medically complex adult and geriatric population.
This position conducts virtual patient visits, with each interaction requiring strong clinical assessment, decision-making, and the ability to manage complex medical needs remotely.
Reporting to the Medical Director, The NP will evaluate and manage a broad range of clinical concerns that may arise between home health visits, including urinary tract infections, wound-related concerns, medication and symptom management, laboratory orders, transitional care management, changes in condition, and end-of-life or palliative care needs. While the position is virtual, previous hands-on field experience is critical to success.
The ideal NP understands what can realistically be assessed and managed in the home, knows how to navigate the complexities of home-based care, and can use clinical judgment to determine when additional services, testing, field intervention, or escalation are appropriate.
Client model combines remote nurses, providers, technology, and AI-supported clinical care to provide proactive support between home health visits, identify changes in condition early, and help patients remain safely at home.
QUALIFICATIONS: ANP/FNP, BC, DEA, with 2+ years of experience providing home/community-based care; complex medical and palliative care experience required, ability to work virtually.
Current and unrestricted Nurse Practitioner license • ANP or FNP accepted
Board Certified
Minimum 2 years of Nurse Practitioner clinical experience
Demonstrated confidence practicing independently and making clinical decisions with minimal direction
Physician collaboration available when needed; candidate should nevertheless be comfortable functioning independently within their scope of practice
Strong clinical assessment and diagnostic reasoning skills
Home care / home-based clinical experience required
Experience caring for medically complex adult and/or geriatric patients
Experience with complex care management, transitional care, chronic disease management, or high-acuity patients strongly preferred
Palliative care and/or hospice experience
Experience managing patients with multiple chronic conditions and complex symptom presentations
Comfortable evaluating patients virtually and determining when additional in-person intervention is required
Strong understanding of how home-based patients present differently from patients seen in traditional office settings
Strong communication skills and ability to establish trust with patients and families virtually
Comfortable collaborating with RNs, physicians, home health clinicians, pharmacists, and interdisciplinary care teams
Technology savvy and comfortable working within virtual care platforms and EMR systems
Strong clinical judgment, prioritization, and escalation skills
Experience with Medicare, Medicare Advantage, Medicaid, or other complex/high-risk populations preferred
